Details of Experience:
Mrs. Donna Bayes arrived at Quality Center for Rehabilitation and Healing on December 16th, 2021, after a five-day stay at Vanderbilt Wilson Medical Center due to a fall, which caused her to have surgery on her right humerus and right hip. Donna came to Quality Center needing 24/7 skilled nursing care, occupational therapy, and physical therapy. The patient was admitted with pain, decreased strength, activity tolerance, balance, increased risk for falls, and decreased mobility.
Upon Mrs. Bayes arrival, she was greeted by several professional team members. Within her first 24 hours, Mrs. Bayes met with clinicians, therapists, our physician, administrative staff, and other members of our team, who welcomed her and answered all questions. Donna was also greeted by the Director of Concierge, Kay Weir, who presented her with a CareRite welcome bag and her warm attentiveness, which she provides to all of the Quality residents and families.
Mrs. Bayes therapy assessment was performed by her therapist, Lisa Spencer. The patient was a one-person assist for sitting up in bed, toileting, and transfers. She was non-weight bearing with the extensive assistance of staff. Donna and the therapist set up goals to maintain and improve functional status while minimizing any functional declines.
Donna was an extensive assist x1 with all of her daily living activities. She made significant progress in the last two weeks. In the beginning, she was non-weight-bearing with the extensive assistance of walking, standing, and transfers. Before coming to the community, she was not able to walk. She has now improved bed mobility from max assist to contact guard assist, and transfers also improved from max assist to contact guard assist. She can ambulate back and forth to the bathroom. She has improved from being non-weight-bearing to independent with a Hemi walker, walking 30 ft to 60 ft, and plans to return back to the assisted living facility at the end of January.
Mrs. Bayes is very impressed with her progress with help from our therapy dept, her techs, and encouraging nurses. She wanted to give a huge thank you to Ashley Matney & Mekinzie Mitchell, her techs, Whitney, her favorite nurse, Maddie Foster & John Bolger therapist, and the entire team at Quality Center.
She shared she has no complaints and would highly recommend our community.
