Details of experience:
Mr. Charles Deshler arrived at Quality Center for Rehabilitation and Healing on January 24, 2022, after a previous stay at Centennial Medical Center, Nashville, Tennessee, with muscle weakness and respiratory issues, due to Covid-19. Charles came to Quality Center needing 24/7 skilled nursing care, occupational therapy, speech therapy, and physical therapy.

Upon Mr. Deshler’s arrival, he was greeted by several professional team members. Within his first 24 hours, Charles met with clinicians, therapists, our physician, administrative staff, and other members of our team, who welcomed him and answered all questions. Charles was also greeted by the Director of Concierge, Kay Weir, who presented him with a CareRite welcome bag and her warm attentiveness, which she provides to all of the Quality residents and families.

Jennifer Young performed Charles’s therapy assessment. The patient was a one-person assistant for sitting up in bed, toileting, and transfers. Charles is at high risk of falling. The staff therapist set goals, such as strengthening, regaining balance, and gait to return the patient to being self-sufficient so he could return home with his lovely wife, Martha.

Charles has made significant progress since he came to our community on February 4. Occupational Therapy updated his progress to standing with tolerance, and for bathing, he only needed contact guard assistance. At this time, the main concern was safety awareness. He was moving fairly well with standby assist with transfer, walking 75 ft while still working on fall prevention and impulsive movement. He was encouraged to use an assistive device. Speech therapy documented improvement in his cognitive and disorientation, but he didn’t feel he was ready to go home. Therapy continued to work with him for another week.

Mr. Deshler worked diligently with therapy to get home. His ambulation improved from 50 ft at evaluation with balance deficits to 200 ft with no use of an assisting device…only supervision. His 02 level was now at 92%, dynamic balance improved, and risk of falls also improved from the risk of falls on 18/28 – to low risk on 24/28 with plans to discharge on 2/11/22.

Charles was pleased with therapy and all they did for him. He stated all the staff here had been very good to him and was very helpful. Our social work department set all his future doctor appointments and set him up with Adoration for home health. No equipment was needed.
