Details of Experience:
Mr. Groce arrived at Quality Center for Rehabilitation & Healing on January 5, 2021, from VA Nashville Falls. Mr. Groce was a referral to Quality for 24/7 skilled nursing care, occupational and physical therapy due to being a high risk for falls, weakness, and weight loss. Upon Mr. Groce’s arrival to Quality Center for Rehabilitation, he was greeted by the amazing clinical team, within his first 24 hours.
Mr. Groce met with our administrative staff, clinical physician, therapist, and the Director of Concierge Services, Kay Weir. She welcomed him with a CareRite welcome bag. She also explained the services she provided and answered all questions.

Mr. Groce’s assessment was performed by the therapist, Chandell Hackett. His balance was 2.0 and his gait 4.0 bilateral in both legs, making him a high fall risk. Mr. Groce needs assistance with his walker for transfers and assistance with all daily activities, currently. The therapy team set goals to help Mr. Rural plan to get back to normal daily activities, such as bed mobility transfers, bathing/dressing, and toileting.

Mr. Groce was steady with his progress of treatment throughout his stay. He went from needing maximum assistance to minimal with bed mobility, transfers, and gait with a rolling walker from 5 ft to 250 ft. He also went from high risk of falls to moderate risk. Mr. Groce is very thankful to the entire therapy team and all the caring staff who had taken care of him during his stay at Quality Center.

He is looking forward to his discharge date home on February 10, 2021, to return home to his wife. He will continue PT/OT with his home health nurse. He has a hospital bed, bedside commode, and a power chair.
