Details of Experience:
Mrs. Laura Hannah arrived at Quality Center for Rehabilitation and Healing on January 27, 2021, from St. Thomas Rutherford Medical Center. Mrs. Hannah was referred to Quality Center for 24/7 skilled nursing care and therapy due to weakness from pneumonia and COVID-19. 

Upon Mrs. Hannah’s arrival at Quality Center, she was greeted by several members of our professional team. Within the first 24 hours of her stay, Mrs. Hannah met our Administrative Team, Clinical Therapist, Physicians, and other members of our fantastic team, who welcomed her warmly. She was also greeted by the Director of Concierge, Miss Kay Weir, who presented her with a CareRite welcome bag after explaining her services and answering any questions she had.

Therapist Chandell Hackett performed Mrs. Hannah’s assessment. She was very weak from Covid & Pneumonia. Ms. Hannah was mid-assist with all her ADL’s (activities of daily living) and with her transfers and mobility, dressing, and toileting. The patient could only achieve 15 minutes of daily activities for a few weeks without taking a break. Therapy and Mrs. Hannah then set goals to enhance her strength and return her to normal independent living. Before discharging home, Mrs. Hannah had progressed from mid-assist to limited supervision. She was ambulating 300 ft with a walker, dressing, bathing, toileting herself independently. 

Mrs. Hannah was discharged home with a walker for home use until she was able to walk independently. She was highly impressed with the overall care she received from everyone at Quality Center. She thanked everyone.
