Mr. W arrived at Quality Center for Rehabilitation and Healing on March 28, 2025. He was previously a resident at St. Thomas Medical, Nashville, Tennessee. He was treated there for pneumonia and was intubated, with gastrointestinal bleeding and extreme body weakness.
Upon Mr. W’s arrival, he was greeted by several members of our professional team. Within his first 24 hours, he met with our clinicians, therapists, physicians, administrative staff, and other team members, who welcomed him to our community and answered all his questions. He was also greeted by the Director of Concierge, Kay Weir, who presented him with a CareRite welcome bag while explaining her role as the liaison.
Upon being admitted to our community, Mr. W met our therapeutic team. He was evaluated by all disciplines, and service goals were established. At the time of evaluation, he had severe weaknesses, decreased balance, and activity tolerance, requiring the need for maximum assistance to total dependence with all his activities of daily living and functional mobility. Mr. W was willing to do whatever it took to get better. He worked hard for months, making some improvements.
Unable to return home, he decided to remain long-term. At discharge from short-term care on 7/5/25, he was able to perform bed mobility with setup assistance, transfers with moderate assistance, and ambulating up to 20ft using a front-wheeled walker. He also showed improvement with occupational therapy for all his activities of daily living. After a little time, Mr. W decided he wanted more therapy to improve his functional status. He worked extremely hard for the next three months and made tremendous improvement.
Mr. W is now able to walk 75-100 ft with a front-wheeled walker, go up and down four stairs, and perform transfers independently. His biggest goal was to be able to dress himself and toilet himself independently with consistent dedication. Mr. W accomplished this goal. He continues to go to the gym daily and has been working hard since being discharged from therapy services. He remains one of therapy’s most dedicated residents and friends in our community. He is such a joy to be around and has given half of the staff in every department a nickname.
Mr. W constantly reminds us how much he loves our team and our community, and that he wouldn’t want to be anywhere else. He is very grateful to everyone who has helped him on his journey, giving a huge shoutout to our therapist, Cindy & Will. Mr. W and I visit daily and have the best time.
