Details of experience: 

Mr. Dale arrived at Quality Center for Rehabilitation and Healing on February 21st, 2024. Following a hospital stay at Nashville General. Due to generalized body weakness, mainly in his left lower extremity. He was referred to by our community for physical therapy and skilled care.

Upon Mr. Dale’s arrival, he was greeted by several members of our professional team. Within his first 48 hours, he met with our clinicians, therapists, physicians, administrative staff, and many other team members, who welcomed him to our community and assured him they were all there to assist him. He was also greeted by the Director of Concierge, Kay Weir, who presented him with a CareRite welcome bag and explained her role as a liaison.

Dale was evaluated by therapy. Physical therapy noted that upon his evaluation, Dale transferred from bed to wheelchair with moderate assistance. He was able to walk 15 feet with a walker. Therapy set Dale at 5 days per week until he was discharged on April 10, 2024. At this time, he was independent with all activities and ambulating over 300 feet, not requiring any assistive devices, with plans to return to the group home where he lives. Unfortunately, Dale had multiple stays with us over the following year. Dale was re-admitted on May 30, 2025. He agreed at this time to remain long-term.

Throughout his time here, he was always willing to participate in therapy and did what was required of him to improve. In January 2026. He set a goal to find a group home and started working harder every day with therapy. He worked tirelessly to achieve his goals. Dale started pushing his wheelchair instead of wheeling around in it. Before you knew it, he was asking to walk halfway around the community with a roll-aide with supervision and assistance from our therapy team. Within two weeks, Dale had achieved his goals and was discharged from therapy on February 16, 2026, performing all tasks independently. He was able to walk unlimited distances throughout the community and climb up and down steps; his safety awareness had improved, and he was ready to start his new adventure. We wished Dale the best and enjoyed working with him.

Dale was discharged to a community support home on February 17, 2026. Home health was set up, and continues therapy for strengthening. All medication was called in to the pharmacy, assuring that Dale understood all information given to him. He was very happy with his stay here and thanked everyone for all their support. We wish him continued success on his healthcare journey.
