Details of Experience:
Mr. Percy W. arrived at Quality Center for Rehabilitation and Healing, on August 21, 2020 from a stay at Vanderbilt University Medical Center in Nashville, Tennessee. Mr. W. was referred to Quality Center for IV antibiotics, wound care, occupational therapy and physical therapy. Percy had an open wound to his left lower leg, displaced oblique fracture of the shaft and humerus in his right arm stemming from his incident.

Upon Mr. W.’s arrival, he was greeted by several members of our professional team. Within his first 24 hours, Percy met with clinicians, therapists, his physician, administrative staff, and others, who welcomed him and answered all questions. Mr. W. was also greeted by the Director of Concierge, Miss. Kay Weir, who presented him with a community welcome gift and her warm attentiveness, which she provides to all of Quality’s residents and families.

Mr. W. had his assessment performed by Chandelle Hackett, he was moderate assist x 2 for standing, sitting up in bed, sitting on side of the bed, and for transfers, he scored a 1- 28. He is at high risk of falling, being non weight bearing on his left leg and right arm. Despite his inability to use those limbs, he continues to push himself to get better by working daily with the therapy team.

Percy arrived at the community with non-weight bearing orders, needing extensive care X 2. The patient is now able to do his ADL’s after about three weeks of therapy. Mr. W. went from 50% non-weight bearing transfers to 25% stand by assist, with increased range of motion in his leg and ankle. Percy set a goal to get stronger with therapy so he can go home, and he has done so. Mr. W.’s balance has increased to 12-28 moderate with walker; and he is now able to transfer from his bed to chair with no assist and walking 20 feet with standby assist.

Percy will need some limited assistance upon his discharge home with his daughter, Carletta on 10/07/2020. He has been scheduled to get assistance at home and continues therapy until he is strong enough to be independent again.

He shared that he is incredibly pleased with results from our therapy department and thanks the entire staff for helping him to get this far in his journey.
